Who is covered:
                The club and its members while participating in club or organization sponsored events. Applies
                in US, its territories or possessions, and Canada.

Insurance Certificates
                Facilities must have certificate for records. The request for certificate is due by October 31th.
                Each member is required to submit payment in only one club. New dancers are free and have
                30 days to change clubs or after they graduate.

                No certificate is needed at demos, street dances, or parades. The club is required to submit a
                "Notification of an event / group travel" form.

How claims are filed
                Facility request payment for damages by letter along with bills.

Accident: Medical Insurance
                What are the limits?
                               $10,000 Usual & customary accident medical expenses
                               $10,000 Accidental death benefit
                               $10,000 Accidental dismemberment (2)
                               $5,000 Accidental dismemberment (1)
                This is a secondary medical insurance and handles covered medical expenses within 52 weeks of
                accident. Max. of $10,000 for eligible expense.

Who is covered
                Club members attending any regularly scheduled and sponsored dance activity in the world.
                This includes group travel. Group travel.
                Group travel is 10 or more members in a vehicle commercially licensed for transport of people.
                Must be operated by a person with a valid operator's license and be 25 or more miles one way.

What is covered
                Accidental bodily injury sustained by an insured person while participating in dancing activities
                sponsored by a club.

Club Accident Report - How to present a claim
                Request a claim form and submit completed form through channels.(Usually the local
                Association) The insurance company will contact the claimer.

What is not covered
                List in tri-fold brochure - eye glasses, hearing aids, false teeth, picnic trips, camping trips,
                snow trips, and use of private cars, babysitting or any dance where alcohol is consumed.
